Clinical Observation & Documentation

Required foundation. Document ≥10 patient contacts with observed vital signs, clinical findings, and your clinical reasoning notes. Signed off by a registered nurse or clinical educator. Clinical observation precedes clinical procedures in all standard nursing curricula — you observe before you do.

Elective. Document structured history-taking (SOAPIE or equivalent format) for ≥3 patient encounters with supervisor feedback on communication quality. Patient details are anonymised. A registered nurse or clinical educator must provide the sign-off.

Medication Administration Competency

Required. Supervisor-signed competency assessment covering all Rights of Medication Administration (right patient, right drug, right dose, right route, right time, right documentation). Supervisor must be a registered nurse or clinical educator. Self-verification and peer verification are NOT accepted — this is the nursing safety core.

Elective. Document ≥5 different supervised procedures (wound care, catheterisation, venepuncture, IV cannulation, nasogastric tube insertion) with a supervisor competency rating for each on the official placement assessment form. Nursing Care Plan Design

Elective. Design a comprehensive care plan (NANDA nursing diagnoses, SMART goals, interventions with rationale, evaluation criteria) for a specific patient scenario reviewed by a qualified nursing educator. The scenario must be specific — a generic 'patient with hypertension' care plan is not sufficient. The proof is a supervised learning exercise, not a care plan to be implemented.
